Cardiomyopathy can be complex, but understanding it doesn’t have to be. EMPOWERCARDIOMYOPATHY™ offers easy-to-understand education and real-life stories from patients who’ve been there. Whether you’re newly diagnosed or managing long-term care, this program helps you feel more in control and connected to a community that cares.

The EMPOWERHF™ Program is designed to guide patients on their journey with heart failure through support, education, tools, and resources to help them every step of the way. The EMPOWERHF™ program seeks to equip heart failure patients with the means and support they need to confidently regain control over their health and lives. Components of the EMPOWERHEARTFAILURE™ program include Welcome Home Heart Failure, MyHeartVisit® Accredited Visiting program, and monthly support group meetings held the 2nd Tuesday of every month at 5 PM EST (email info@mendedhearts.org to receive the link to join the next meeting).

Managing cholesterol is about more than numbers—it’s about your overall heart health. EMPOWERCHOLESTEROL™ gives you the knowledge and encouragement you need to make informed choices. Learn how lifestyle, medication, and support can help you take steps toward a healthier future, one decision at a time.
EMPOWERCHDTM
EMPOWERCHD™ is proud to include the Mended Little Hearts Welcome Home CHD program. This special program is for families who have a child with a congenital heart defect (CHD). It gives families helpful information and kind support during their CHD journey.
Families can also join the Parent Matching program. This connects parents whose children have similar heart conditions via email. It helps families feel understood, supported, and part of a caring community.

The EMPOWERCKD™ program educates and informs patients and caregivers on the connection between chronic kidney disease and cardiovascular disease, treatment options available and tools and resources for these patients.

This program helps people learn about heart failure and how it connects to other health problems like kidney and metabolic diseases. It shares easy-to-understand videos and information to help you learn about your condition. The program also connects you with others who understand what you’re going through and gives you tools and support to help you live well and take care of your health.

Diabetes and heart health are closely connected. EMPOWERDIABETES™ helps you understand that connection and gives you tools to manage both. With practical tips, emotional support, and guidance from others who’ve walked this path, you’ll feel more confident in your ability to live well and protect your heart.
EMPOWERHEARTATTACKTM
This comprehensive support and education program activates patients to learn more about surviving a heart attack and avoiding other CVDs and health conditions that stem from the health scare, educates patients through patient journey videos and educational resources, and connects patients to peer support, tools, and information to support them as they learn to live with and manage their condition.

EMPOWERHYPERTENSIONTM
The EMPOWERHYPERTENSION™ Program also aims to mitigate challenges faced by people living with hypertension to help them live longer, healthier lives by creating and sharing resources that help them manage their symptoms, adhere to guideline-directed medical therapy, embrace regular exercise and a healthy diet, understand their treatment options, and, most importantly, equip them with knowledge about their condition.

The EMPOWERMETABOLIC™ program helps you understand heart failure and how it’s linked to metabolic diseases like diabetes, obesity, and high blood pressure. These conditions can affect how your heart works by causing inflammation, energy imbalances, and damage to blood vessels. The program offers simple videos and easy-to-read materials to help you learn more about your health. It also connects you with others who share similar experiences and gives you tools and support to help you feel better, stay active, and take care of your heart.

EMPOWERSTROKETM
This program equips stroke survivors with the tools, knowledge, and confidence needed to navigate life after stroke and prevent future complications. The Stroke Peer Support and Education Program is committed to easing the challenges stroke survivors face while supporting them in leading longer, healthier lives.

EMPOWERTRANSPLANTTM
The EMPOWERTRANSPLANT™ program is designed to enhance patient preparedness and post-transplant life by integrating peer support, educational resources, and caregiver training, thereby extending the continuum of care beyond the clinical environment.
