The EMPOWERHF™ Program is designed to guide patients on their journey with heart failure and CKM syndrome through support, education, tools, and resources to help them every step of the way. The EMPOWERHF™ program seeks to equip heart failure patients with the means and support they need to confidently regain control over their health and lives. Components of the EMPOWERHEARTFAILURE™ program include Welcome Home Heart Failure, MyHeartVisit® Accredited Visiting program, and monthly support group meetings held the 2nd Tuesday of every month at 5 PM EST (email info@mendedhearts.org to receive the link to join the next meeting).
