Supporting Teen’s Transition to Adult Care

MLH aligns itself with the Adult Congenital Heart Association (ACHA). in its policy that all adults with CHD receive care from a cardiologist who specializes in CHD. This is also set out in the guidelines from the American College of Cardiology and American Heart Association.

For more information on helping children transition to specialized adult care, visit the ACHA website. On this website, there is even a directory of centers where there is specialized adult CHD care. In addition, they have a travel directory for adults with CHD who are traveling or moving to other areas in the United States.
